Affinity Health Center is a fast-paced, growing community health center focused on providing patient-centered care with compassion, dignity, and respect. We serve all people, regardless of income or insurance status. Affinity Health Center seeks a Medical Case Manager to support patient care and services. The Medical Case Manager (MCM) will use strong relationship building skills to partner with patients to deliver services that lead to satisfactory health outcomes for patients living with HIV. Medical case management services include assessing patient needs, connecting to resources, addressing barriers to care, working with the patient to create goals, and providing ongoing support to achieve goals. The MCM will assist patients with applying for and maintaining benefits such as Medicaid, Medicare, ADAP, and health insurance through the marketplace.

- Bachelor’s or Master’s Degree in Social Work preferred
