Under the supervision of the Case Management Supervisor, the Outreach Case Manager is responsible for coordinating and developing the outreach program at Affinity Health Center. The Outreach Case Manager assists in linking new or returning patients to care and services; and prevents existing patients from falling out of medical care.
In addition, the Outreach Case Manager is responsible for delivering services that lead to satisfactory health outcomes for people living with HIV/AIDS in Affinity Health Center’s service area. The Outreach Case Manager will ensure that services are delivered in accordance with the SC Ryan White Part B Case Management and Ryan White Part A Case Management guidelines. Outreach Case Manager participates in Continuous Quality Improvement activities of the organization and the Performance Management System.
Flexible hours and travel for home visits and/or meetings. Some evenings may be required.
