Affinity Health Center is looking for experienced Family Nurse Practitioners or Physician Assistant to deliver compassionate health care and to partner with our patients on chronic care management. Under the supervision of the Medical Director, the Family Nurse Practitioner/Physician Assistant is a member of the patient care team, responsible for providing patient-centered primary care for Affinity patients. Under appropriate supervision, the Midlevel plays a key role in extending the physician’s services. The Midlevel Provider will be responsible for compliance with government regulations related to the provision of healthcare. The Midlevel Provider participates in the Continuous Quality Improvement activities of the organization and the Performance Management System.
Salary: $95,000 – $110,000
