The Colorectal Cancer Prevention Network (CCPN) at the University of South Carolina (USC) is a successful functioning individual entity under the College of Arts & Sciences. We are steadfast with our mission as we work to reduce colorectal cancer through awareness, education and screening. As part of these ongoing efforts, the screening program continues to expand its reach to maximize efforts in increasing screening delivery to medically underserved individuals in our state. Innovative initiatives have been launched by the CCPN that fostered colorectal cancer awareness, education and screening by implementing strategies and solutions aimed to reduce the incidence of colorectal cancer and to also address the rise in young adult (<45yrs/age) cases of colorectal cancer. We are looking for a Bilingual (Spanish/English) candidate who is organized, and self-motivated with interest in providing patient navigation services in the Upstate region of South Carolina.  Candidate must be a resident of South Carolina with preferred medical office/medical field experience.  This position is home-based, and the candidate must be able to provide a working space that meets CCPN’s home-based working policy.  This position also requires travel to an assigned geographical area. 

The Patient Navigator duties include medical chart review, patient interviews by phone and in person, education visits at the patients’ medical home, coordination of visits, and follow-up communications with patients and providers.  Patient Navigator candidate will be required to:  

  • Be bilingual (fluent in English/Spanish)  
  • Possess a good understanding of medical terminology  
  • Assist in obtaining resolution to patients’, clinics’, partners’ questions 
  • Build working relationships, solve problems, and support participants to navigate the health care system if additional care is required post screening.  
  • Assist patients with barriers to care to achieve CRC screening.  
  • Establish effective, trusting working relationships with patients to ensure continuous engagement.  
  • Build professional relationships with the healthcare team of referring clinics, gastroenterologists, and endoscopy centers.  
  • Possess a valid driver’s license, personal vehicle and car insurance.  
  • Travel within the assigned geographical area as needed based upon schedule.  Navigator will receive a monthly travel reimbursement 
  • Possess strong computer and communication skills and the ability to use various software. Computer and software will be provided.  
  • Possess and utilize iPhone and a contract with local carrier for unlimited data and hot spot capabilities to meet HIPAA requirements set forth by CCPN. Navigator will receive a monthly cell phone stipend.
